This registration form is required for each child attending an Early Intervention Early Childhood Special Education program. It is completed at the child's initial eligibility, prior to entering the program, and then again annually at the beginning of each school year in September. The registration form provides essential information regarding the child's demographic information, parent contact information, emergency contact information, and physician information. The completed form is filed in the child's permanent file in the main office and in the classroom file. It is crucial that when changes occur throughout the school year that the form and the child's files be updated immediately.
